What to Expect at Your First Visit

Your first appointment is about getting to know each other — not about selling you a treatment plan.



Here's how it usually goes:

We'll confirm your info, go over your health history, and answer any questions you have about paperwork.

X-rays and exam

Dr. Candido will take a careful look at what's going on in your mouth, using 3D imaging when needed to see the full picture.

Conversation 

You won’t get a lecture in our office. Instead, you’ll have a real back-and-forth conversation with Dr. Candido, where he explains what he's seeing, you ask whatever's on your mind, and together you figure out a plan that makes sense.

Most first visits take about 60-90 minutes.

Don’t Forget to Bring These!

When you come to your first visit, we need a few documents.

List of any medications you're currently taking

Note: We see patients 18 and older. For pediatric dentistry, we recommend finding a specialist who focuses on kids.

Patient Forms

New Patient Form

The New Patient Registration Form collects essential information  including personal details, contact information, and dental history. It helps Dr. Candido understand where you've been and what you need, so your very first conversation is already informed and personal.

Authorization For Medical And Dental Treatment(S)

To provide you with the best possible care, we ask for a signed authorization that allows our team to perform the necessary diagnostic procedures, examinations, and treatments. This keeps everything transparent and ensures you're always informed.
